Social service professionals are available to all patients and families. Staff members assist you with social and emotional needs while at the hospital. They also provide assistance when you’re leaving the hospital, if you need follow-up care or services.

- Preadmission screening for nursing home placement.
- Discharge planning.

- Financial counseling.
- Outpatient continuity of care.
- Patient and family conferences.
- Information about support groups.
- Crisis intervention.
While at the hospital, you can ask to speak with a social service professional. They will gladly help you find answers and solutions to your concerns.
